Summary
Spike is being tormented by the spirit of a brutal surgeon named Matthias Pavayne. Since Pavayne's magic helps him stay out of Hell, he wanders around Wolfram & Hart cleaning up the loose spirits. With growing concern for Spike, Fred tries frantically to find a way to make him corporeal, and prevent him from being sent to Hell. While Spike battles it out with Pavayne, Fred arrives with her new contraption. As she is attacked by Pavayne, Spike is faced with a choice; he can save Fred, or he can use her machine to become corporeal. In the end, Spike chose to save Fred by pushing Pavayne into the machine to make Pavayne corporeal. A corporeal Pavayne thus lost all his power and got entombed in the W&H prison for all eternality. As Angel puts it, "Welcome to Hell."

Quotes and trivia
- In this episode, Simon Templeman plays the role of Matthias Pavayne, a spirit battleing a non-corporeal vampire. In the Legacy of Kain video game series, he provides the voice for the main character Kain. Throughout the series, Kain (a vampire himself) also battles a vampire who can become non-corporeal.
- It may be interesting to note that, when this episode was aired on the WB, that it was forewarned as a particularly violent episode before its showing. Indeed, because of the sadistic nature of the mad surgeon and the ghostly depictions he provided to Spike, it was a much gorier episode of Angel than usual.
