In the fictional universe established by Buffy and Angel, the Buffyverse, a Hellmouth is an area of massive supernatural activity.

Explanation
Hellmouths are places of increased supernatural energy. According to the mythology of the "Buffyverse", this is the area in which the barriers between dimensions are weak. The Hellmouth has a focal point which serves as a portal between earth and the underworld. For these reasons, the Hellmouth attracts demons and other supernatural creatures, becoming a "hot spot" for supernatural activity.Opening the Sunnydale Hellmouth
The Sunnydale Hellmouth lies underneath the library of Sunnydale High school.
There have been many attempts by apocalyptic individuals/groups to open the Sunnydale Hellmouth, such as in the episodes "The Harvest" and "The Zeppo". In season 7, The First Evil was going to open it to bring forth the Turok-Han, but Buffy and the potential slayers descend through the portal in the series finale, "Chosen" to fight them. Spike uses a magical amulet to destroy the Turok-Han and close the Hellmouth. The ensuing cataclysm not only collapses the Hellmouth, but swallows the entire town of Sunnydale along with it.
The Hellmouth in Cleveland
There are two confirmed Hellmouth locations in the Buffyverse canon, one in Sunnydale, CA and one in Cleveland, OH.
The location of another Hellmouth is first released in the episode "The Wish". In an alternative timeline, Giles mentions that "there is a lot of demonic activity in Cleveland." Also in that timeline, Buffy Summers serves as Slayer at this Hellmouth without the friendship of Xander and Willow.
Giles mentions the second Hellmouth in "Chosen".
In the non-canon novel by Queen of the Slayers, which takes place after "Chosen", Faith and Robin Wood go to Cleveland to control the supernatural activity there.
Trivia
- Influence: This concept of "hellmouth" is lately being furthered, probably by other writers, and was recently used to refer to an ancient artifact, in the SCIFI TV movie Phantom Force.
