- A Natural History Group, which is often referred to as a trial's "third arm", is the name given to the group of subjects in a drug trial that receive no treatment of any kind whatsoever -- and, therefore, their condition is left to run its "natural" course -- and the observed outcomes with this group are compared with the outcomes manifested by a group that has been given the active drug, and that manifested by another group who have been given a dummy, placebo drug (thus, the "third arm").
