The abolished Kosel municipality had 1 369 inhabitants in 2002 and 1 759 in 1994. The present day municipality has 55 749.
The main ethnic group are Macedonians, who make up 85% of the population, followed by Albanians (5.9%), Roma (0.1%), Serbs (0.6%), Turks (4%), Vlachs (0.5%), Aromanians (0.50%) and other make up 4.2% of the population.
The municipality has 29 inhabited places, one town and 28 villages.
