First miniseries

Issue #1

As of the first issue of the first miniseries, it appears that Tony Stark is a genius because of an accident his mother suffered while she was carrying him in her womb; he has undifferentiated neural tissue throughout his body, as if his whole body itself is a brain, giving him tremendous mental capacity: quicker in thought than any human in history, but at the cost of constant chronic pain similar to third-degree burns due to the overstimulation of the neural cells that make up his skin. His father uses his newly invented biological armor to ease Tony's pain.

Issue #2

In the second issue, a young Stark (appearing in the comic to be a child of about 8-10 years, but revealed to be 4, indicating that aside from the constant pain and high-level intelligence dropped on him by his birth, his body also experiences rapid growth) is shown covered with the blue skin-armor developed by Stark's father in the previous issue (as indicated he would need to wear to keep him sane by his pregnant Mother in the previous issue). Since Stark, in his appearances in The Ultimates, appears with a normal skin color, it can be presumed that by that point he had developed a skin-colored or transparent version of the protective armor. As well, the inoperable brain tumor that the older Stark possesses can be shown to be a result of his rapidly mutated genetics (and, the fact that he apparently did not possess said tumor until his middle-age can in itself be considered to be a small miracle).

Issue #4

However the creation of the Iron Man suit has nothing to do with Tony Stark's condition. In the fourth issue Tony states that he made the suit to protect his father, but later shows the suit as a weapons project designed for Stark industry.

Issue #5

There are two variations of the suit at the end of the miniseries. One is a suit roughly 6 feet tall designed for more practical testings, the other is a large suit at least 20 feet tall for designing any additions to the traditional suit before including them to the more restricted design. As of the fifth issue, the suit doesn't fly, but hovers or bounces and is not completely stable.

Tony Stark appears to still be an alcoholic just as he has been in the past, regardless of his young age. Shortly after his father's arrest, for suspicion of being involved with the gruesome murder of his enemy Zebediah Stane, he gets a taste of champagne at a party held by Tony's father. The alcohol affects his brain more than a normal human due to the brain tissue which is distributed through out his entire body. Later on, Tony mentions that he drinks alcohol to dull the pain which he constantly feels throughout his body.
